Semaglutide

Approved weight-loss and diabetes drug

Evidence briefing
Semaglutide is FDA-approved as Ozempic, Rybelsus and Wegovy in product-specific injectable and oral forms. A semaglutide research-vial product is not one of those approved products.
Key limitation
FDA approval and evidence are product-, route- and indication-specific; they do not establish equivalence of research-vial material.
Regulatory context
FDA-approved in the US as Ozempic and Rybelsus for type 2 diabetes and as Wegovy for weight management and other label-specific indications, including cardiovascular risk reduction in some adults with overweight or obesity and noncirrhotic MASH. Approved products include injections and tablets. Approval applies to the labeled drug products, not research-vial semaglutide.
Reported risks
Nausea, vomiting, diarrhea and constipation are common. Less common: gallbladder problems, pancreatitis, and muscle loss alongside fat loss. Boxed warning for thyroid C-cell tumors seen in rodents. Measuring errors with self-mixed vials have sent people to poison control.

FDA-approved

Tirzepatide

Approved weight-loss, diabetes, sleep-apnea drug

Evidence briefing
Tirzepatide is FDA-approved as Mounjaro for type 2 diabetes and Zepbound for chronic weight management and obesity-related sleep apnea; 2026 Mounjaro labeling adds cardiovascular risk reduction for a defined high-risk type 2 diabetes population. Research-vial products are not the approved products.
Key limitation
Approval does not establish the identity, purity or equivalence of research-vial products.
Regulatory context
FDA-approved in the US as Mounjaro for type 2 diabetes, including pediatric patients 10 and older, and for reducing major cardiovascular events in adults with type 2 diabetes at high cardiovascular risk; and as Zepbound for chronic weight management and moderate-to-severe obstructive sleep apnea in adults with obesity. Approval applies to the labeled products, not research-vial tirzepatide.
Reported risks
Similar to semaglutide: GI upset, possible gallbladder disease and pancreatitis, and the rodent thyroid-tumor warning. Can reduce the reliability of birth-control pills around dose changes.
